It was shortly after 10:30 p.m. when police were called about a man shot in the 1900 block of South Fairview Street.

There they found an unresponsive man with multiple gunshot wounds to his upper torso, police said. Firefighters pronounced the man — identified Tuesday as Froylan Santos of Santa Ana — dead at the scene.

Homicide investigators were working to determine what led to the man’s death. No arrests have been reported.

Mustafa Qadiri, 38, of Irvine was named in a federal grand jury indictment returned Wednesday, May 5, charging him with six counts of money laundering, four counts of bank fraud, four counts of wire fraud, and one count of aggravated identity theft.

Prosecutors say Qadiri claimed to operate four Newport Beach-based companies: All American Lending and All American Capital Holdings were purportedly mortgage businesses, while RadMediaLab and Ad Blot were advertising businesses.

In May and June 2020, he submitted false Paycheck Protection Program loan applications to three banks on behalf of those companies, according to the indictment.

Authorities allege that in the applications, Qadiri changed bank account records with inflated balances, gave false information about how many employees were paid wages, and gave fake quarterly federal-tax return forms.

He is also accused of using someone else’s name, Social Security number and signature to apply for one of the loans.

None of the businesses are operating, according to the indictment, and the banks funded the PPP loans and transferred funds into the accounts Qadiri controlled.

Those loans were made available through the CARES Act, designed to provide emergency funding to help Americans suffering from the economic effects of the COVID-19 pandemic. Part of the funds were forgivable loans to small businesses to retain employees and for other expenses.

Once Qadari received the money, he bought luxury cars such as a Ferrari, a Bentley and a Lamborghini, federal authorities said, and he also went on lavish vacations and paid for other personal expenses.

Federal agents have since seized the cars along with $2 million from his bank account.

An individual familiar with the woman and two men called authorities around 1 p.m. about a possible homicide at the home on Red Apple, a street in a gated community off Alton Parkway and Royal Oak, said Irvine police Sgt. Karie Davies, spokeswoman for the department.

Officers arrived at the home and found the woman’s body, police said. Also at the home were two men who officers detained for questioning in what investigators are viewing as a homicide, Davies said.

Davies confirmed the woman, two men, and the 911 caller knew each other, but declined to say who they were and their exact relationship to one another.

Coroner authorities would have to confirm the cause and manner of the woman’s death, Davies said.

The woman is believed to be a resident of the Red Apple home, Davies said. Detectives are unsure whether the other two men were visiting or also residents.

Detectives said they believe there are others who may live at the home but are not suspected to be involved in the incident.

While detectives were interviewing the two detained men at the Irvine police station, whose identities were not released, they had not been arrested or booked on any specific charges as of early Tuesday evening. Police said they are sure no others were involved in the woman’s death.

Authorities warned residents of a heavy police presence but said there was no threat to the area. The community is near a large, busy shopping center.

Two women in the Mercedes left the scene, authorities said, but were found at a nearby hospital being treated for injuries.

The crash, just before 3 a.m., occurred on Brookhurst Street between Broadway and Orange Avenue.

First responders found the cars with front-end damage. The Mercedes had a small fire under its hood, which firefighters quickly extinguished.

Inside the Kia were the three men, who appeared to be in their 20s and were pronounced dead, Anaheim Sgt. Shane Carringer said.

Kevin Lee was coming out of a nearby club when the crash happened.

“I just heard a car screeching and then an impact sounds, and I saw a bunch of debris and cars,” Lee told a news photographer with OnScene.TV, which works with The Orange County Register.

He ran toward the crash and saw injured people getting out of the Mercedes.

“Then a fire started, and so we grabbed fire extinguishers and tried to take the fire out,” he said.

The victims have been identified by the police; however, their names are pending notification of family, Carringer said.

Carringer said that after the crash, a good Samaritan drove the two injured women to a local hospital.

The women remained hospitalized Tuesday evening while investigators continued to piece together the events leading up to the deadly crash, Carringer said.

He said the crash was more complex than usual and will take longer to sort out.

For one thing, witness statements that appear to conflict with those involved in the collision, he said.

The manner and circumstance under which the two women left the scene of the crash also must be determined, Carringer said.

“The women left the scene, but is it fleeing the scene of the accident?” Carringer said. “We still don’t know if they were in need of medical attention or if they were even conscious when they were taken to the hospital.”

He also referenced a surveillance video of the crash that appears to show the Kia Soul stopping in the roadway in the path of the Mercedes Benz. While the Mercedes would normally have the right-of-way, he said the Mercedes’ speed could play a factor in responsibility for the crash; however, that would be a difficult detail to learn quickly, Carringer said.

“It’s difficult to judge the speed just off the headlights (during the night); it’s easier in the daytime,” Carringer said.

Police ask the good Samaritan or anyone else with information about the crash to call them at 714-765-1861.

Just before noon on Friday, April 30, firefighters were called to a fire in the 11800 block of Beach Boulevard, said Greg Barta of the Orange County Fire Authority.

Once the blaze was out, firefighters searched the building and found the person dead inside. Fiire Authorities on Monday, May 3, did not have more information about the individual.

The investigation into what sparked the blaze was ongoing.

The fire grew and spread between two houses, one of which had another building in the backyard, O’Connor said.

A dog was killed, and, in all, some 20 people were displaced, he said.

The blaze was first graded as a two-alarm fire but then was upgraded to a 4-alarm one just before 2 p.m., meaning that several more fire units were called for backup.

As around 80 firefighters battled the blaze, three of them were shocked by electrical lines, O’Connor said. The shocks were not serious, but they were taken to hospitals as a precaution. A fourth firefighter was also hospitalized, for heat exhaustion.

O’Connor said all the firefighters were in good condition. Two other people were treated on scene for smoke inhalation and also were in good condition, officials said.

Fire crews were up against Friday’s high temperatures, low humidity and brisk wind, which was already going to pose a fire risk throughout the region. The National Weather Service predicted Orange would reach a high of 92 degrees on Friday.

It took firefighters about one hour to get the blaze under control. It primarily damaged two houses next to each other, leaving them blackened.

Chris De Rosas, a resident of one of the two damaged homes, waited across the street from behind yellow tape with his family.

He was in the house next to the one where the blaze started, which he shares with his parents, sister, brother and cousin.

De Rosas’s sister was showering when she saw the smoke from the window and ran out to notify the others.

“I actually saw the flames in my face,” he said. His parents weren’t home at the time but the rest of them escaped unharmed.

“We got out just in time,” De Rosas said.

Miguel Lopez, who lives across the street, saw De Rosas’s family running out of their home.

Shortly before, he’d been in his front yard when he noticed the smoke.

“I thought it was a barbecue at first,” Lopez said. “Then I saw the smoke turn black.”

Lopez, one of the 911 callers, tried to steer people clear of the burning area. He said he heard pops during the fire, and De Rosas said he heard a small explosion.

The cause of the fire is under investigation, O’Connor said.

Karen Marie Dondanville, 53, of Mission Viejo faces dozens of charges including grand theft, insurance fraud, financial elder abuse, and forgery.

She was arrested on Friday, April 23, and, court records show, on Monday, April 26, she pleaded not guilty to or denied the counts.

Authorities say her scheme resulted in her collecting $183,047 in stolen premium payments.

Insurance department authorities received multiple complaints from customers about Dondanville while she was doing business under the names Streamline Insurance Services and Broadstreet Insurance Services.

Dondanville tried to cover up the fraud by giving clients phony insurance documents as proof they were covered, the agency said.

Earlier this year, the Department of Insurance said, it issued cease-and-desists orders to Dondanville after receiving complaints from customers when she was doing business without a license, which had been revoked in November 2019.

Dondanville billed yet didn’t fully pay insurance companies, resulting in many policies lapsing for nonpayment, the insurance department said. She also submitted fraudulent documents to insurance companies, diverting premium refunds to herself, the insurance department said.

“This left her 32 victims with more than 10,000 collective days without insurance coverage, exposing them to the danger of substantial uninsured losses,” the department said in a statement.

In one case, she is accused of creating fake insurance documents charging a homeowner $2,800 but the true cost of the policy was $900 and she paid the insurance company only $400.

Another customer unknowingly without coverage experienced approximately $100,000 in uncovered losses, the Department of Insurance said.

The Orange County District Attorney’s Office is seeking restitution, with a criminal complaint showing prosecutors have calculated the figure at $335,349.

Insurance authorities remind consumers to verify that their insurance agents are licensed and can do so online on the California Department of Insurance’s website.

The department asks those who believe they are a victim of an agent or someonce acting as one to file a complaint with the department.

Shortly after 11 p.m. Juan Barajas and his wife were in the living room of his house in the 1400 block of West Borchard Avenue when he saw fire outside. The garage was ablaze.

“I just saw a big splash of orange,” Barajas told a news photographer. “We just started getting the kids, I got four kids and we started taking them out (of the house) and that’s when we called 911.”

Smoke was creeping into the rooms of the home, Barajas said, but everyone was able to make it outside safely. Once outside, he tried to put the flames in the garage out with a hose.

Firefighters arrived and were able to extinguish the fire in about 12 minutes, said Orange County Fire Authority Battalion Chief Mike Summers. They also helped the family find their dog, who was apparently hiding in the back part of the property.

 

 

The fire started inside the garage, which was closed at the time, and did not spread to the rest of the home, OCFA Capt. Sean Doran said. Fire investigators believe it was sparked by a malfunction in one of several electrical devices near the blaze’s point of origin

Barajas said it looked like someone intentionally set the blaze.

“I was checking my cameras and we noticed someone running out of the property,” he said.

In the home’s camera footage shared with reporters, a hooded figure is visible leaving the home through an opening in the gate and running off on the sidewalk. However, “the individual in that video was not involved in the fire,” Doran said. That conclusion  was based on analysis of the recording, evidence collected at the scene and interviews with witnesses.

The identity of the person in the video remained unclear Thursday evening.

On Friday, April 23, Hugo Standley Vargas climbed the fence into the cemetery around 4 a.m., Santa Ana Cpl. Anthony Bertagna said.

He went into an area where veterans are interred and broke into a columbarium, a structure that encloses urns, Bertagna said.

The Santa Ana resident was discovered by a cemetery worker who called police. When officers arrived, they found him in the fetal position, Bertagna said, near an opened vault.

“Apparently one of them was damaged and there was an urn out of it and he was laying among the ashes,” Bertagna said.

When questioned, Vargas told officers he was at the cemetery to visit an uncle who was dead.

“But that person whose ashes were out had no relation to (Vargas),” Bertagna said.

It was unclear if Vargas has a relative interred at the cemetery, he said.

Apart from breaking into the urn enclosure, police said, he damaged a flag pole, taking down an American flag.

On Monday, Vargas was charged with single felony counts of destruction of a grave, vault, niche or crypt, vandalizing a cemetery, and vandalism with damages costing more than $400.

He pleaded not guilty.

It was unclear what city he had lived in. The LAPD said it was investigating what motivated Lopez to shoot at people.

His cause of death had not been released, in particular whether he was fatally shot by at least one officer or shot himself. But the LAPD said he was shot at and suffered multiple gunshot wounds.

The victims who were killed, Los Angeles coroner officials said, were Alexis Carbajal, 24, and Mingzhi Zhu, 42, of South Pasadena.

In the early hours of Tuesday, April 27, police said, Lopez went on a shooting spree in Exposition Park and downtown Los Angeles.

Just before 1 a.m. Tuesday, Lopez drove up next to a vehicle near Exposition Boulevard and Figueroa Street and shot at the first victim, a man grazed in the head and expected to survive, police said.

Lopez then approached a newlywed couple waiting in a car in the drive-thru lane at a Starbucks at Figueroa and 28th streets, police said, with Carbajal exchanging words with him before he was shot multiple times.

About 20 minutes later, Lopez’s Jeep Cherokee pulled up next to a Cadillac Escalade at Figueroa and 7th streets, where he fired into the driver’s side window, police said, killing Zhu.

Police spotted the Jeep, and a pursuit began, with police saying he shot again and missed a driver and then hit a Tesla when apparently firing at police.

During a lengthy pursuit a spike strip was laid out in his path, and the Jeep came to a halt on the 91 Freeway in Fullerton. He remained inside.

Police tried unsuccessfully to negotiate with him, using loudspeakers, and brought in one family member to communicate with him. Their attempts to talk to him ensnarled traffic for hours.

Shortly after 5 a.m., SWAT deployed tear gas into the Jeep and Lopez shot at officers, police said, prompting them to return fire. He was found dead in the sport-utility vehicle, where a weapon was recovered, police said.

Carbajal’s eldest sister, Cindy Partida, said her relatives came together to support each other immediately after learning what happened.

At the Starbucks where he was killed, a memorial was created of flowers and candles. Mourners gathered at the site on Tuesday evening.

Carbajal’s family was “heartbroken to get the news at the scene,” Partida told the Southern California News Group in an Instagram message on Tuesday. She described her brother as an avid gamer and “the biggest dork ever, who made everyone laugh.”

Carbajal is survived by his parents, his wife and three sisters. His family has created a GoFundMe campaign to help cover his funeral expenses.

“We all feel like this is a nightmare that we can’t wake up from,” Partida said. “We wish we could have him back.”

In South Pasadena, neighbors and friends rallied around the family of Zhu, the second man killed,  said Yuki Cutcheon, president of the South Pasadena Chinese American Club. Members of the community arranged for the delivery of meals to his widow, 8-year-old son and 2-year-old daughter, and more than $130,000 had been donated to a GoFundMe campaign set up to support them.

They moved to Southern California from China in 2016, and have lived in South Pasadena for 2 years, Cutcheon said. Zhu drove for Uber and was the sole source of income for his family. He was less than a minute away from picking up his next fare and just trying to earn a living when he was gunned down, Cutcheon said.
